SUMMARY The goal of this position is to support individuals served with severe and persistent mental illness, serious emotional disturbance, and/or intellectual/developmental disabilities to become active and integrated citizens within their communities.
Assists in fostering the highest possible quality of life for individuals in the community, by providing supports and skill building consistent with the Plan of Service to increase independent functioning and decreasing the need for treatment in more restrictive settings.
This will be accomplished by helping the individual to identify their personal preferences and implementing a plan of support which honors these preferences.
Staff in this position will work in cooperation with individuals, families, community members, case holders and consultants as identified through the Person Centered planning process.
Persons with Lived Experience with mental illness, intellectual/developmental disabilities or substance abuse disorders are encouraged to apply.
JOB DUTIES AND RESPONSIBILITIES Provide services and supports to the individuals as specified in the Plan of Service.
Document contact and services, complete billing process per policy and procedures.
Work in collaboration with other agency programs and community agencies to serve the needs of the consumer as identified in the Plan of Service.
Must work a flexible schedule i.
e.
weekends, evenings, and holidays as assigned.
Travel as directed to meet individual and administrative needs.
Participate in committee work (inter departmental, inter agency) as directed.
Responsible to assure the maintenance of departmental case records in compliance with DCH, CMH, Medicaid and all other applicable standards.
Facilitate consumer groups as assigned in a manner consistent with Plans of Service.
Effectively provide support during all phases of the individual recovery process.
Provide beneficial education, coordination, and support for all individuals in treatment.
Act as an advocate for all individuals served through the program; work cooperatively with all staff, families, service agencies, and community members to promote an overall positive and productive attitude.
Work a flexible schedule which will include weekends, evening, and holidays determined by the need s of the individual.
Work with consumers and CSM to develop a schedule of activities as identified in each plan of service including but not limited to community inclusion, skill building, self-advocacy and other skills in support of their recovery journey.
Ability to manage account for, and document individuals money and turn in all receipts to appropriate person in accordance with Agency guidelines.
Must work as a member of a treatment team and take directives from case holders and Supervisors in order to meet Agency and plan of service needs.
Ability to successfully implement plans of service for each individual; maintain required documentation to verify consumer progress and attainment of individual goals.
Teach and assist with activities associated with meal planning and preparation .
Teach and assist with financial management including paying of monthly bills, budgeting, and shopping.
Teach and assist individual with activities associated with maintaining personal hygiene, laundry, perform household cleaning as determined by the needs of the consumer.

Teach and assist consumers in accessing support within the community as identified within Person Centered Plan.
Teach and assist consumers in medication management as identified in the plan of service through prompting, education, and monitoring.
Assist and teach consumers how to organize activities, com muni ty and socialization, and with peers.
Teach and assist consumers with completing forms necessary to maintain Medicaid, subsidized housing, food stamps, social security, and housing.
Other duties as assigned.
COMPETENCIES Able to provide beneficial skill and recovery facilitation on a group or individual level.
Knowledge of Medicaid regulations, Level of Care Guidelines, and Person Centered Planning Guidelines.
Ability to work with fellow staff, consumers, and other agency personnel in a collaborative and beneficial manner.
Demonstrates the ability to present one’s self, the agency and the individuals we serve in a positive manner at all times.
Holds knowledge of and can effectively operate the EHR and email Agency email system Full knowledge and application of Recipient Rights and HIPAA regulations.
Full knowledge and application of recovery principles.
Possession of a driver's license valid in the State of Michigan Vehicle insurance as required by Michigan Law Good driving record.
