Clinical Case Manager

*Position Summary: *Assess, plan, organize, and work with patients to help identify, assist and/or coordinate needs, goals and objectives.
Develop a specific program plan for patients to meet assessed needs.
Monitor, document and report progress toward goals.
*Tasks:* 1.
Clinically assess and document the needs of each client.
2.
Complete an individual treatment plan with the client.
3.
Work with the client to meet the treatment goals and document progress after each session.
4.
Reviews and evaluates available resources and services from local agencies to provide assistance for clients.
5.
Complete appropriate review with client input.
6.
Complete discharge process.
7.
Refer to other programs or individuals as clinically indicated.
8.
Develops program plan, keeps records, prepares reports 9.
Be available for client in case of an emergrency.
10.
Abides by and coordinates with referral source according to the guidelines set forth by referring source.
11.
Attends monthly group meetings with administrative staff and clinical supervision meetings with Site Supervisor as applicable.
12.
Other duties as assigned.
Job Type: Full-time Pay: $16.
