PER VISIT
Summary:
This individual is responsible for skilled services under the supervision of a Registered Nurse (RN) and in accordance with the Plan of Care and with the agency policies; prepares clinical progress notes, assists the RN in performing specialized procedures, prepares equipment and materials for treatments and assists the patient in learning appropriate self-care.
The RN delegates skilled nursing functions after evaluating the complexity of the nursing activity and the level of problem solving required.

Responsibilities: Assists with coordination of patient care with other members of the home care team.
Provides problem solving skills to assist in coordination of daily scheduling.
Participates in patient care conferences and Team meetings,allowing for the exchange of information in both formal and informal networking systems.
Provides skilled nursing care to patients in accordance with the Plan of care under the supervision of an RN.
Performs delegated nursing activities which are appropriate to the LPN’s preparation and experience as evidenced by skills checklist and in accordance with agency policies and program guidelines.
Contacts providers for clinical orders,referrals and treatments as indicated.
Document and implement direction as appropriate.
Implements the plan of care, following the RN’s initial assessment and development of the plan of care, utilizing practice standards.
Conferences with the RN on a regular basis, collects specific assessment data, participates in the planning,evaluation and modifying of the nursing care plan as appropriate.
Provides for emotional and physical comfort and safety of patients.
Recognizes and understands the effects of social and economic problems with patients.
Works with Home Health Aides (HHA’s) and other disciplines as appropriate.
Maintains records in accordance with agency requirements.
Participates regularly in agency staff meetings, team meetings and inservices.
Participates on agency committees or special task forces as assigned.
Participates in development , revision and implementation of policies and procedures.
Assumes responsibility for personal and professional growth.
Demonstrates excellent customer service.
Contributes to organization success targets for patient satisfaction and not operating margin.
Maintains confidentiality of patient and agency information.
Responsible for special projects and other duties as assigned.
Other information: EDUCATION/EXPERIENCE Current licensure with the Michigan State Board of Nursing.
BLS certification.
Graduation from a school of practical nursing NAPNES recognized.
Two years of nursing experience following graduation or at the discretion of the Program Services Supervisor.
Homecare experience preferred.
KNOWLEDGE/SKILLS/ABILITIES Knowledge in current nursing practice, ability to relate positively to patients, families and co-workers.
Ability to read, write, speak and comprehend English.
Second language desirable but not required.
Knowledge of basic computer programs.
Valid Michigan Driver’s License.
Excellent communication skills, both verbally and written.
Comfortable with the use of the computer program, and other software systems used within the VNA system.
Demonstrates problem solving skills.
Ability to type.
Near visual acuity and functional hearing required.
Successful ability to independently access any type of a private residence.
Ability to lift greater than 20 pounds.
Reliable transportation and proof of current Insurance.
WORKING CONDITIONS/PHYSICAL DEMANDS Ability to maintain regular, punctual attendance consistent with the ADA, FMLA and other federal, state and local standards.
Constant talking, hearing tasting/smelling, near vision, midrange vision, far vision, depth perception, visual accommodation, color vision and field of vision.
Frequent sitting, twisting, reaching, handling, fingering and feeling.
Occasional standing, walking, lifting, carrying, pushing, pulling climbing, balancing, stooping, kneeling, crouching and squatting.
Occasional lifting 0-100 lbs.
