Job Summary:
We are seeking a highly skilled and motivated Clinical Pharmacist to join our team.
As a Clinical Pharmacist, you will play a crucial role in providing pharmaceutical care to patients in a hospital setting.
Your expertise in pediatrics, acute pain management, critical care, and medication administration will be essential in ensuring the safe and effective use of medications.
Duties:
- Review and interpret medication orders for accuracy and appropriateness
- Collaborate with healthcare providers to develop and implement patient-specific medication therapy plans
- Monitor patients' response to drug therapy and make recommendations for adjustments as needed
- Provide drug information and education to healthcare professionals, patients, and their families
- Conduct medication reconciliation to prevent medication errors
- Participate in multidisciplinary rounds and provide clinical pharmacy services to various patient populations
- Prepare and dispense medications, including IV infusions, using aseptic technique
- Ensure compliance with all applicable laws, regulations, and standards of practice
Requirements:
- Doctor of Pharmacy (Pharm.
D.
) degree from an accredited school of pharmacy
- Completion of a post-residency clinical pharmacy training program or equivalent experience
- Current pharmacist licensure in the state of [LOCATION]
